Consent Forms

This document must be complete prior to your first massage therapy session.   You can either print it, complete it online, or fill it out in person.

Conditions for Booking

We require all first-time clients to make a reservation deposit in order to book your first appointment.  All subsequent appointments will not require a reservation deposit.

Our booking system sends out multiple email and text message reminders several days prior to your scheduled appointment. We ask that you please reschedule or cancel at least 24 hours before the beginning of your scheduled appointment.
  • You may reschedule your appointment to avoid the cancellation fee of $45.00
  • Same-day cancellations at least 2 hours before scheduled appointment may be rescheduled once without being charged a cancellation fee
  • Same-day cancellations within 2 hours before scheduled appointment may be rescheduled once but will be subject to a cancellation fee
  • Clients with good attendance records and/or extenuating circumstances may be exempt of any cancellation fees

Our booking system sends out email and text message reminders several days prior to your scheduled appointment.

In case of a late arrival to your scheduled appointment and if time permits, we will try to accommodate your full session duration.

In case of a no show to your scheduled appointment, we may charge you the full amount of all services scheduled.
